This patient had an extracapsular cataract extraction (ECCE) 4 years ago which was complicated by posterior capsule rupture and vitreous loss. An anterior chamber intraocular lens (ACIOL) was inserted but the haptic of the ACIOL was in contact with the corneal endothelium causing corneal decompensation. The ACIOL was subsequently removed (three sutures still visible). There is an irregular corneal reflex and corneal haze caused by edema. The pupil is distorted.
