| Pupil-involving Third Nerve Palsy | |
| The right pupil is dilated in this 53-year-old man who reported the acute onset of headache and complete closure of his right eye. The pupil was non-reactive. Associated findings in the right eye included complete ptosis, exotropia, hypotropia, and impaired adduction, elevation and depression. An isolated, pupil-involving third nerve palsy should immediately prompt consideration of an intracranial aneurysm, particularly when pain is present. Tumors at the skull base are also in the differential diagnosis. | |
