Homonymous Hemianopia, Occipital Lobe, Macula Sparing
This is a left homonymous hemianopia with macular sparing. The preservation of macular function in the otherwise blind hemifields is thought to be related to the dual blood supply of the occipital lobe. Although the posterior cerebral artery constitutes the main blood supply, a branch of the middle cerebral artery supplements the circulation at the occipital pole. The latter represents the macula topographically. In patients with an occipital stroke from a posterior cerebral artery occlusion, the contribution from the middle cerebral artery may prevent infarction of the pole, thus preserving macular function.
