Choroidal Melanoma with Liver Metastasis (Histology, Colour Photography, Ultrasound)

Patient: 52 years of age, male, BCVA Light Perception, IOP 25 mmHg. Ocular Medical History: red eye since several months, vision loss at OD. General Medical History: MRI revealed liver metastasis. Main Complaint: red eye and vision loss at OD. Purpose: to demonstrate histology and clinical view of late choroidal melanoma with liver metastasis. Methods: histology, colour image anterior segment, ultrasound. Findings: Colour Image Anterior Segment: conjunctival and episcleral hyperemia, flat anterior chamber, visible intraocular pigmented tumor. Ultrasound: choroidal lesion with basal dimension of 11 mm and a mean thickness of 8.5 mm. Histology: secondary angle closure glaucoma, complete retinal detachment, intracular and subretinal lesion with transscleral infiltration, complete necrosis within lesion. Discussion: Following the authors of (1) and (2) choroidal melanoma is the most common primary malignant intraocular tumor in adults, with a 10-year cumulative metastatic rate of 34%. The most common site of metastasis is the liver (95%). Median survival after liver metastasis is poor, being around 4-6 months, with a 1-year survival of 10%-15%. The current treatment of metastatic choroidal melanoma is limited by the lack of effective systemic therapy. Options for the management of the primary intraocular tumor include radical surgery as well as conservative treatments in order to preserve visual acuity.
