Idiopathic True Exfoliation of the Lens Capsule (Colour Photography)

Patient: 91 years of age, male, BCVA Rt: 6/9+3, Lt: 6/9, IOP: Rt :18, Lt: 15 Ocular Medical History: nuclear sclerosis Rt & Lt, bilateral tilted discs, on treatment with G Ganfort once a day to Rt & Lt eye, G Azopt BD Rt & Lt eye General Medical History: Med Hx nil contributory. Purpose: to present true exfoliation of the lens capsule. Methods: Colour Photography. Findings: Colour Photography: delamination and curling of the superficial layer of the lens capsule. Discussion: True exfoliation of the lens capsule is rare. It is characterized by delamination and curling of the superficial layers into the anterior chamber. The underlying risk factors could be exposure to excessive heat, infrared radiation, inflammation, trauma, and idiopathic. Idiopathic true exfoliation has been reported in the elderly patients, which seemed to be the case in our patient. This condition is different from the Pseudo exfoliation of the lens capsule in which fibrillar material is deposited on the lens, zonules, and trabecular meshwork, this can be associated with glaucoma. While doing a cataract extraction, Trypan blue staining of the capsule is suggested to avoid creating a partial thickness capsulorrhexis. Literature:-.
