Duane's Syndrome
The primary deviation in Duane’s syndrome can vary from patient to patient. This patient is exotropic in primary gaze. Note that the examiner must bring the child's head to the primary position to avoid missing a deviation that may be hidden by a subtle face turn. With attempted adduction, the globe retracts causing upper lid ptosis. The eye also shoots downward. Vertical deviations are common in adduction. In this case, abduction is normal although it may be limited in some patients.
