This 73-year-old woman experienced acute, painless visual loss OD three weeks ago. Her examination reveals a visual acuity of 20/100, altitudinal field loss, and an afferent pupillary defect on the right. The right optic disc, pictured here, exhibits mild swelling and pronounced hemorrhage. Note that subtle optic atrophy has developed in the interval since the onset of symptoms, which explains why the disc no longer appears hyperemic despite the residual swelling.
