Slitlamp examination shows in an otherwise relatively clear lens a large cleft between nucleus and cortex (arrows). This 50 zear old patient had blood sugar levels over 400 mg/dL. Patients complain of changing refraction. The cleft disappeared after therapy of the diabetes
