In normal individuals, the temporal aspect of the disc nearly always appears slightly lighter in color than the nasal aspect of the disc. Therefore, it can be very difficult for the clinician to distinguish between physiologic temporal disc pallor and mild pathologic temporal pallor. The right optic disc shown here appears to have mild-to-moderate pathologic pallor, a conclusion supported by the paucity of visible nerve fiber layer reflexes in the papillomacular region and elsewhere. However, to be certain, comparison with the fellow eye is required.
