This patient suffered from decreased vision (20/400) for several years. A large macular hole is visible. The hole is surrounded by a fairly broad rim of the same color as attached retina. This rim again is surrounded by granular appearing yellowish deposits. The vitreous is completely detached from the posterior retina. The retina is attached in the rim area (see OCT), an unusual finding. We interpret the finding as an older macular hole which originally had a surrounding detachment as the ring of accumulated macrophages indicates. Later the retina reatttached.
